Social workers are available at OCH to assist and assess patient and family needs in a number of different situations.  Examples include:  financial questions or concerns, insurance difficulties, limited social support, interpersonal relationship difficulties, young family, difficulty coping with diagnosis.  OCH social workers also assist in discharge planning to make your transition from hospital to home, nursing home or assisted living a smooth one.  We are patient advocates and provide education on a number of different topics.  These are examples of possible situations that patient and family may have difficulty handling on their own.  For more information, or if you have questions or concerns, please contact:  Sherri Hietbrink at (712) 754-2574.
